You can also follow the steps described below to complete and submit the form by mail.

Make sure you have the latest version of Adobe Reader™ installed on your computer. If not, you can get a Free copy Here from Adobe.com website    

1) First click the link below and the Enrollment Form will open in Your Browser if PDF Format.

2) Place your mouse/cursor in the lower right corner of the page. You will see a PDF bar appear. Or depending on your computer system and your version of Adobe or whatever type PDF reader you have, it may be in the upper left corner. Main thing is just save a copy to your computer.

3) Click the save disk that is located between the printer icon and the magnifying glass with the (+) symbol in it. A box will pop up on your computer asking you where to save the form at. ( Save As ). Pay close attention where you save the form because you will need to go there and open it.

4) Locate the Enrollment Form that you saved on your computer and double click the PDF file.

5) Complete the form and print or save it to your computer

6) Then print/mail the form to the address above.
